Owner/ Head Physical Therapist
Meet Seamas, co-owner and head physical therapist at Strongbridge Physical Therapy. Seamas brings a diverse background to his role, receiving a double major in Geology and Anthropology/Sociology from Lafayette College and initially working in environmental remediation as a Geologist. However, drawn by a calling to directly impact lives in a meaningful way, Seamas transitioned to the field of Physical Therapy.
Seamas pursued his passion for physical therapy at the MGH Institute of Health Professions, where he earned his Doctorate of Physical Therapy. He further honed his skills under the mentorship of Jodi Llacera Klein at Cape Ann Physical Therapy in Manchester by the Sea, MA, before founding Strongbridge in 2023. He is a member of the American Physical Therapy Association (APTA) and has received his Orthopedic Clinical Specialist (OCS) certification, a designation awarded by the American Board of Physical Therapy Specialties (ABPTS) to physical therapists who have demonstrated advanced clinical knowledge, skills, and experience in the field of orthopedic physical therapy.
As a licensed, board-certified orthopedic specialist, Seamas brings a wealth of knowledge and experience to every session. However, what truly sets him apart is his deeply ingrained philosophy of patient-centered care. Seamas is dedicated to helping members of his community rediscover their ability to engage in activities they love and lead healthier lives. His approach is rooted in compassion, listening attentively to each patient's unique needs and crafting personalized treatment plans tailored to their goals.
Outside of work, Seamas loves spending time with his wife Hilary and their four boys, Seamas, Soren, Fletcher, and the eagerly anticipated arrival of Terrance in May. He enjoys kiteboarding, snowboarding, and relishes moments in his yard with his kids, engaging in activities such as crafting maple syrup, tending to their chickens, or piloting remote-controlled airplanes.

Hilary (Carroll) McGuire, a native of Seacoast New Hampshire, takes great pride in opening Strongbridge Physical Therapy alongside her husband, Seamas. Together, they look forward to providing an invaluable service to their local community.
With a background in oncology nursing, Hilary developed an appreciation for the impact of even small improvements in functionality, witnessing firsthand how they can bring joy and empowerment to her patients. Transitioning to the role of a Palliative Care nurse practitioner, she has remained steadfast in her commitment to enhancing her patients' quality of life, irrespective of their life or illness circumstances.
At Strongbridge Physical Therapy, Hilary is eager to extend these core values and skills. She approaches clinic development with a holistic perspective, recognizing the interconnectedness of physical, emotional, and social well-being. Additionally, she embraces a community-centered approach, understanding the importance of fostering strong relationships within the local area.
Hilary holds a Bachelor of Science degree in Neuroscience from Lafayette College, followed by a Bachelor of Science in Nursing from Johns Hopkins University. She further pursued doctoral training at the University of Washington, supported by the American Cancer Society Graduate Scholarship in Cancer Nursing Practice. Hilary is also a graduate of the Harvard Interprofessional Palliative Care Fellowship.
In addition to her commitments at Strongbridge Physical Therapy, Hilary maintains an active role in the healthcare community as a part-time Palliative Care Nurse Practitioner at Brigham and Women’s Hospital and Dana Farber Cancer Institute.
When not working, she loves spending time with her husband and their (almost!) four sons, Seamas, Soren, Fletcher, and Terrance (due in May!). You may also find her spending time with friends, biking, or kayaking on the Hampton Falls River behind Strongbridge Physical Therapy.

No need to worry about submitting superbills to your insurance company—we offer direct-to-payer courtesy billing for out-of-network plans. This means that we handle the billing process for you, even if your insurance is out-of-network, so you don’t have to deal with the paperwork.

Sometimes, you may need to pay part of your cost upfront, especially if you haven’t met your deductible or owe a co-pay. We’re happy to provide you with a cost estimate before your visit.

Other Payment Options
We offer flexible cash-based payment options. We also offer payment plans if needed. Sometimes, paying cash can be a more affordable choice, especially if you have a high-deductible insurance plan.
We are also happy to accept Health Savings Account (HSA) and Flexible Spending Account (FSA) payments, making it easier for you to manage the cost of your healthcare using pre-tax dollars. Simply use your HSA or FSA card at the time of your visit, just like you would with a credit or debit card. We also provide a detailed receipt for your records, which you can keep for tax purposes or in case your HSA or FSA provider requests proof of eligible services.

Understanding How Insurance works
We know insurance can feel complicated, but we’re here to make it simple! Health insurance helps cover part of the costs when you visit doctors, physical therapists, or need medications. Here’s a quick guide to how it works, plus some info about out-of-network coverage that could help you get the best care possible.

In-Network Providers: In-network providers, like our clinic, have an agreement with your insurance company to offer lower rates. This means your insurance will cover more, and you’ll pay less out of pocket. Once you meet your deductible you may still need to pay a portion, like a co-pay (a set amount per visit) or co-insurance (a percentage of the cost), but it’s usually less than seeing an out-of-network provider.

Out-of-Network Providers: Out-of-network providers don’t have an agreement with your insurance, which might make it seem like they’ll cost more. However, using your out-of-network benefits may give you access to more personalized, high-quality care that can actually be more affordable in the long run.

At our clinic, we believe in providing 1:1 treatment with a Doctor of Physical Therapy (DPT) in private rooms, ensuring you get the focused, expert care you deserve. Unfortunately, some in-network contracts don’t allow us to offer this level of care due to restrictions on treatment times and reimbursement rates. Using out-of-network coverage gives us the flexibility to provide the best treatment possible, often with fewer visits.

We live and work here, and as your neighbors, we want to ensure that you get the best possible treatment without sacrificing quality.

We want to make accessing top-notch care as simple as possible. That’s why we offer direct-to-payer courtesy billing to help reduce the stress of managing insurance claims. No more worrying about submitting superbills to get reimbursed by your insurance… We’ll handle the paperwork and submit the claims directly to your insurance provider on your behalf. You’ll still receive reimbursement based on your out-of-network benefits, making it easier for you to afford the care you need. Many patients find that they pay less overall for the same high-quality care, and with fewer restrictions on treatment, we can often address your needs in fewer visits. Out-of-network coverage gives you more control over your healthcare and allows us to customize your treatment without being limited by insurance guidelines.
How Using Out-of-Network Might Save You Money
While out-of-network care may initially seem more expensive, it could be more cost-effective in the long run. Here’s an example:
Let’s say a clinic in-network with your insurance offers 30-minute sessions (15 min of which is provided by a Physical Therapy Assistant), and your insurance covers 80% of the $100 visit cost. You pay $20 per visit, but because the treatment time is shorter and less personalized, you might need 15 visits to fully recover. Your total out-of-pocket cost is $300 (15 visits × $20).
At our clinic, with out-of-network coverage, we offer 40-minute follow-up sessions at $125 (entire visit 1:1 with DPT). Depending on your insurance, it might cover 60%, meaning you only pay $50 per visit. Because our sessions are longer and tailored to your specific needs, you may require fewer total visits—let’s say just 6 visits to achieve your goals. Your total out-of-pocket cost would still be $300 (6 visits × $50). While the per-visit cost might seem higher initially, you’re getting more value from each session, needing fewer visits, and ultimately receiving more effective care. Plus, with fewer visits needed, you’ll miss less work and get back to the activities you enjoy faster.

You can contribute money to your HSA tax-free, and when you need to pay for qualified medical expenses—such as physical therapy—you can use your HSA funds. HSA funds roll over year to year, allowing your savings to grow if you don’t use them.
Flexible Spending Accounts (FSAs) are offered by employers and allow you to set aside pre-tax dollars for eligible healthcare expenses. Unlike HSAs, FSAs often have a “use-it-or-lose-it” policy, meaning any unused funds may expire at the end of the plan year.
The benefits of using HSAs/FSAs are numerous: HSA/FSAs allow you to use tax-free money to pay for eligible healthcare services, such as physical therapy, co-pays, deductibles, and out-of-pocket costs, reducing your overall expenses. It’s important to verify that the services you're paying for are HSA-eligible. Using your HSA/FSA gives you flexibility in choosing providers—like us—who may be out-of-network but offer the best care for your specific needs. We are happy to accept HSA/FSAs as payment for your cash-based visit, or your deductible, co-pay, or co-insurance for your insurance-based visit.

We take referrals, but you do not necessarily need one, as New Hampshire is a direct access state, allowing you to seek physical therapy services without a physician's referral. While your insurance may require a referral, we'll assist you in understanding your coverage. With direct access, you can start your treatment immediately, saving time and reducing delays in care.
